Athlete Participation Waiver

Assumption of Risk & Release of Liability

I understand that participation in RunLikePro LLC athletic training involves inherent risks, including but not limited to running, cross country, sprint and endurance training, agility drills, plyometrics, jumping, strength and conditioning, stretching, basketball, volleyball, games, outdoor trail running, and related athletic activities. These activities may result in falls, collisions, sprains, strains, fractures, concussions, heat-related illness, or other serious injuries.

I certify that the participant is physically able to participate and agree to notify Coach Natalia Rodionova of any medical condition or injury that may affect participation.

I voluntarily assume all risks associated with participation and, to the fullest extent permitted by California law, release and hold harmless RunLikePro LLC, Coach Natalia Rodionova, assistant coaches, volunteers, facility owners, and affiliated organizations from any claims, liabilities, damages, or expenses arising from participation in RunLikePro LLC activities, except in cases of gross negligence or willful misconduct.

Transportation Authorization

I authorize my child to be transported by Coach Natalia Rodionova in a school van and/or the coach's personal vehicle to and from parks, trails, schools, athletic facilities, competitions, or other approved training locations when necessary for RunLikePro LLC activities. I understand that transportation involves inherent risks and voluntarily accept those risks.

Emergency Medical & First Aid Authorization

If I cannot be reached in an emergency, I authorize Coach Natalia Rodionova to obtain emergency medical treatment for the participant, including first aid, emergency medical services, ambulance transportation, and hospital care if deemed necessary. I also authorize the coach to provide routine first aid, including ice, bandages, athletic tape, and cleaning of minor wounds. I understand that I am responsible for any resulting medical expenses.

Concussion Acknowledgment

I understand that any participant suspected of having sustained a concussion or head injury may be removed from activity immediately and may not return until medically cleared when appropriate. I agree to inform the coach of any diagnosed concussion before the participant returns to training.
